I had a 16t crack and bought 3 replacements, the 1st replacement couldn't hold on and i have had 8-10 packs on the second.
When the updated pinions are out I will install it and stop worrying if this is the flight it will fail.

400 flights are not few flights but still shouldn't wear to that point.
Here's mine in less than 400 flights. It definitely wore out faster than the short shaft adapter which still wore out faster than the 5/32" rod stock I used to custom-make my own short adapter insert.

I've heard a couple cases with 15T slipping but that's because of oil contamination.The 6mm shaft/OWB it runs are much more robust (450-size heli mainshaft OWB). I think the problem with 4mm shaft/OWBs (250-size heli mainshaft OWB) is that it's hard to synchronize the 2 OWBs to even out the load. The top one always grabs 1st and takes the most load.

Hmm both bearings wore evenly on mine but maybe the top did go first and the bottom one was doing all the work until it wore out.

I've been talking the protos 500 Mines stretched with 13 tooth pinion, 95% of my flights were at 2450rpms 12.5* pitch .

If I kept flying that long shaft it'd wear out the bottom OWB location as well.

That shaft has seen some 14T and 13T about 60/40 of its life. I fly my stretch with 470mm blades at 2250-2400 with 11.5-12* pitch.
